How does ICSI work?

In the ICSI procedure, a minuscule needle called a micropipette is used to inject a single sperm into the center of the egg. As the human egg is one-tenth of a millimetre in diameter & the sperm 100 times smaller, this is a very delicate method performed by the highly qualified embryologist under a sophisticated microscope.

ICSI is done as a part of IVF and a technique used during in vitro fertilization. During the ICSI procedure, you will need to take ovarian stimulating drugs and your fertility doctor will monitor your progress with blood tests and ultrasounds. Once your follicles are grown good sized, you will need to go through the egg retrieval process, where eggs are removed from your ovaries with a specialized, ultrasound-guided needle. Your partner will provide his sperm sample the same day. Once the eggs are retrieved, an embryologist will place the eggs in a special culture and using a microscope and tiny needle, a single sperm will be injected into an egg. If the fertilization takes place, and the embryos are healthy, an embryo will be transferred to your uterus, via a catheter placed through the cervix, two to five days after the retrieval.

‘’For around half of couples who are having complications conceiving, the source of infertility is sperm-related.”

Who might be recommended to have ICSI?

  • You have a very low sperm count
  • Your sperm are abnormally shaped or they don’t move normally (poor motility)
  • You’ve had IVF previously and none or very few of the eggs are fertilized
  • You need sperm to be collected through the surgical procedure from the testicles or epididymis
  • You’re using frozen sperm in your treatment which isn’t of the prime quality, especially, if it was stored because your fertility was under threat, or following surgical sperm retrieval
  • You’re having embryo testing for a genetic condition, & sperm sticking to the outside of the eggs would interfere with the results

‘’Whether the issue is a male-factor or not, it’s still worth looking into ICSI and having it with your IVF cycle. It’s very crucial to have a good doctor to learn what’s best for you, based on the results from various tests made.’’

When is ICSI not right for you?

If you’ve had treatment in the past & poor quality or immature eggs meant the sperm and the egg had difficulty fertilizing, then ICSI is doubtful to help.

‘’For most people who have no evidence of male factor infertility, the possibility of getting pregnant are the same even if they have ICSI or not.”

How successful is one cycle of ICSI?

ICSI is very successful at helping the sperm and the egg to fertilize- fertilization happens in around 90% of cases and the ICSI success rate tend to be very similar to IVF.

ICSI has higher chances of succeeding depending on the embryologist’s expertise, the lab quality, as well as the doctor’s skills upon handling the embryo transfer.

How safe is ICSI?

ICSI is the most common and effective treatment and just like any other infertility treatments, it carries a slight risk and it’s rare. As technology continues to advance, so does the process, lessening any ICSI risks today.

ICSI is all about pinpointing the best sperm to collect and inject into mature eggs. Not only can this help improve the fertilization and pregnancy rates, but it also addresses egg-related problems.

‘’ICSI has truly developed in the past 25 years with improved micro-manipulation techniques, more accurate equipment and expertise is also sharpened”.

The first ICSI procedure was done in 1992, with the use of donor sperms to help with success rates, however as more studies and improved technologies come along, donor sperm use has dropped. Today, up to 95% of men can be birth fathers because of continually improving ICSI procedures; the procedure has truly helped with male infertility and more studies and ways to improve ICSI continues.
